A 48 by 40 inch pallet built on three stringers, with deck boards nailed across them. The 48 inch side runs along the stringers. Forks go in from the two 40 inch ends, or from the sides through notches cut in the stringers.
"Grade A" has no single industry definition. On this site and in the Load Line it means a pallet with no broken or missing boards, no repairs that stick out, and stringers that are whole, ready to reuse without work.
